June 16-20 9:00am - 12:00pm | Yay! Jesus Is Our True North VBS is coming soon for ages 4 years old - entering 4th grade. Registration is $15.00 per child and includes a t-shirt. We will have fun with hands-on Bible lessons, crafts, activities, games, skits and songs as we learn that in this world, Jesus is our unmovable, unshakeable forever friend!

Shiloh's Medical Release  please complete this form and upload it, or print and email it to chrissy@shilohcc.com or walk it into our office between 9am - 4pm.
You will need thave a medical release per child, per year.

To Upload--You must downlaod this form to your computer then go back to the upload button on the form--which will bring up your downloaded form. Then attached it.
